Complete the following form to submit for a new rebate. To ensure your submission will be approved, be certain that:

Your purchase is made between April 15 - June 30, 2026.
Your rebate submission is made by July 31, 2026.
There's a clear, identifiable receipt showing qualifying products.
The receipt clearly shows the following information: receipt date, receipt number (if available), product name/number and/or description, quantity, retailer name & address and price. You may submit multiple pages or photos to ensure legibility. Submissions without sufficient documentation will be denied.
Your receipt can only be submitted once.

2. Upload Supporting Documentation

Make sure that you have clearly identified the qualifying products on the receipt (highlight or circle them).
Upload a scan/photo of your receipt. Receipt must show all applicable purchase information (as outlined in the instructions above).
There is a maximum rebate limit of 20 total approved items per person. The 20-item limit may be reached through one submission with multiple items or across multiple submissions (e.g., one item per submission).
You may attach multiple files/images of more than one receipt.
Keep copies of all materials submitted for your records.
